Recommendations to the panel on Reassessment of Risk

Report on John Smith by Dr. Hazel Cole, 17th February 1991

Smith has been in the secure unit for seven years following an aggravated assault motivated by religious mania. At the time he was suffering an attack of acute schizophrenia, manifesting symptoms of paranoia, thought disorder, delusions of persecution and of grandeur.

Currently, Smith has several complex and interacting pathologies and many insecurities carried over from an unhappy childhood. His schizophrenia has been in remission for five years, following successful management with chloropromazine. He has previously suffered episodes of epileptic attacks, linked to a focus in his temporal lobes.

The regime of drugs he takes to control his epilepsy restricts the range and effectiveness of the anti-psychotics he may take. Tricyclics have been effective in the past but following an extensive neurological examination, it was the opinion of the case psychiatrist that this treatment should be stopped.

POSSIBLE HOMOSEXUALITY.

CLEAR EVIDENCE OF PSYCHOPATHOLOGY.

UNSUITED FOR MILITARY SERVICE.

Appendix C –

Summer term report

John Smith, Form five – Form teachers report, Summer Term 1993 John continues to drift through school, paying little attention to his work. He is a cheerful member of the class and gets by comfortably enough but he is wasting his potential. I am not confident that he will do as well as could in his GCSE’s. While my fingers are crossed, I think he should brace himself to be disappointed with his results. A short, sharp shock might be just what it takes to wake him up and start taking things more seriously.
